Job summary

Elliot Health System seeks a Social Worker Case Manager for its inpatient behavioral health unit in Manchester, NH. You will provide clinical interventions with patients and families, promote safe care, and coordinate discharge planning with the care team.

Qualifications

• MSW is required; NH LICSW preferred.
• 1–3 years of health-care experience, hospital setting preferred.
• Excellent clinical documentation and EMR usage preferred.

Responsibilities

• Collaborate with the primary nurse and multidisciplinary team for social work assessment.
• Develop, implement, and evaluate integrated clinical pathways.
• Provide supportive counseling and crisis intervention for patient/family.
• Monitor patient progress to ensure timely care.
• Coordinate unit-level care and act as a resource to staff.
• Intervene to address delays in service affecting care quality or length of stay.
• Collaborate with case manager to assess continued need for inpatient care.
• Participate in quality management activities to improve patient care.
• Contribute to policy/procedure development related to patient care processes.
• Identify organizational priorities for improvement.
• Assess patient/family adaptation to illness and care needs.
• Serve as liaison to community social service agencies for discharge planning.
• Demonstrate ability to address care needs across age groups: adolescent, adult, geriatric.
• Knowledge of outcomes research, managed care, utilization management.
• Familiarity with information systems design and development.
