SolutionHealth in Manchester, NH seeks a Social Worker Case Manager to support adult patients with acute and chronic psychiatric disorders, coordinating care to expedite recovery and discharge.

You will provide clinical interventions with patients and families to maximize well-being and appropriate use of health care services across the continuum, collaborating with the health care team to ensure timely, quality care.

Formación

• MSW required with licensure or eligibility.
• Strong documentation and EMR proficiency.
• 1–3 years in healthcare, hospital setting preferred.

Responsabilidades

• Collaborates with the primary nurse and care team on social work assessment.
• Participates in development and evaluation of clinical pathways.
• Provides supportive counseling and crisis intervention as needed.
• Monitors patient progress to ensure timely care.
• Assists in discharge planning and coordination of care.
• Identifies delays in service impacting quality or length of stay.
• Collaborates with case manager to assess clinical course for acute care needs.
• Engages in quality management activities to improve patient care.
• Contributes to hospital policy revisions related to patient care processes.
• Assesses patient/family adaptation and capacity to provide care.
