Job summary

Spectrum Health Systems in Westboro, MA is seeking a dedicated Case Manager for its inpatient residential treatment program. This full-time, first-shift position provides clinical services and case management to clients with mental health, substance use, or dual diagnosis, on a Tue-Sat schedule.

You will develop, implement, and revise individualized treatment plans, refer clients to community services, document progress, and participate in multidisciplinary team care planning.

Qualifications

• Minimum of a high school diploma; bachelor’s degree preferred.
• At least one year experience in substance abuse treatment or case management.
• LADC I or CADAC certification preferred, but not required.

Responsibilities

• Providing documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems.
• Developing, implementing and revising individual treatment plans for assigned clients in established time frames with client participation.
• Referring clients to appropriate services in the community as necessary.
• Participating in multidisciplinary team treatment planning, case review and case conferences.
