• Conduct psychosocial assessment and develop an effective treatment plan with patient safety in mind.
• Obtain information from patient/family interviews to inform treatment planning.
• Provide crisis intervention and assist families in understanding life situations and impacts.
• Participate in multidisciplinary treatment team meetings and discharge planning.
• Identify and coordinate patient needs across illness, hospitalization, finances and community services.
• Promote recovery and social functioning while maintaining documentation and patient records.
