Qualifications

• Master’s degree required in psychology, social work, or counseling.
• Licensure as LCPC or LCSW with Illinois IDFPR is required.
• CPI training within 3 months of hire is required.
• Must be able to use standard DAIR documentation.
• Ability to perform discharge planning and collaboration with the treatment team.

Responsibilities

• Provides individual, group, and family counseling to assist patients/families in achieving treatment goals.
• Provides individual counseling to patients as stipulated on the treatment plan.
• Facilitates at least one regular group therapy per week.
• Ensures families are involved in treatment at least two times per week.
• Provides family counseling as stipulated on the treatment plan (adult/geriatric unit).
• Collaborates with the treatment team and community providers on an individualized treatment plan and discharge planning.
