Peachford Hospital in Dunwoody, GA, a 246-bed acute psychiatric facility, seeks a full-time day shift Master’s prepared Case Manager/Social Worker/MSW to join our interdisciplinary team. Qualifications

• License-eligible master’s degree in a related clinical area is required.
• Previous inpatient/outpatient hospital experience is preferred.

Responsibilities

• Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus.
• Provide individual, group and family solutions-focused interventions.
• Facilitate process and didactic groups.
• Develop an individualized discharge plan utilizing information from the patient, treatment team and family to determine needs.
• Identify family or community resources such as group homes, halfway houses or treatment providers.
