The role includes collaborating with community partners, delivering person-centered care, and updating treatment plans with clients and families.

Responsibilities include conducting assessments, linking clients to resources, and documenting in EMR.

Qualifications

• Education: Bachelor's degree required.
• Experience: case management or behavioral health setting preferred.
• Ability to develop supportive relationships with clients and families.
• Valid FL driver's license with good driving record and insurance.

Responsibilities

• Provide mental health and substance use case management services.
• Refer and link clients and families to additional services.
• Develop and update treatment plans with input from clients and families.
• Enter documentation into Electronic Medical Record in a timely manner.
• Coordinate with local law enforcement and stakeholders to support treatment.
