Job summary

Centerstone’s Access Center Case Manager provides intake assessments and coordinates admissions and discharges for clients in need of referrals to inpatient care or diversion. This role operates within Centerstone’s Behavioral Health Hospital in Bradenton and engages individuals of all ages, including Baker Act and Marchman Act cases.

Qualifications

• Bachelor’s Degree in Behavioral Health or related discipline required.
• Experience in a mental health or addictions setting preferred.
• Valid Florida Driver’s License with safe driving record and auto insurance required.

Responsibilities

• Conducts intake assessments and coordinates admissions and discharges.
• Provides crisis response and care coordination to clients.
• Links clients with appropriate levels of care and services.
• Maintains client records and documents case management activities.
• Works with clients in their natural environment and with families.
• Travel may be required to support services.
