Qualifications

• Active TX Physical Therapist license and professional liability insurance.
• Graduate degree in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patient needs.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in home health.
• Develop and implement individualized treatment plans to restore function.
• Provide interventions including exercise, gait training, manual therapy.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, sessions, and progress using digital platforms.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and prevent re-injury.
