PARS Therapy in Tyler, TX is seeking compassionate Physical Therapists to join a forward-looking home health team. You'll use the Roger app—a smart, HIPAA-compliant documentation platform—to streamline visits, cut paperwork time, and focus on patient care.

As part of a flexible schedule and supportive environment, you’ll evaluate patients, develop individualized plans, educate caregivers, and document progress on digital platforms. Texas license required; prior home health experience preferred.

Qualifications

• Active Texas Physical Therapy license required.
• Experience in home health or rehabilitative settings preferred.
• Strong ability to adapt care approaches to various patient needs.
• Excellent communication with patients, families, and team members.

Responsabilités

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes.
• Accurately document evaluations, treatments, and progress notes using digital platforms.
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
