Job summary

Charles Lea Center is hiring a full-time Innovation Coordinator to support people in Tennessee with a person-centered, employment-first approach and technology-enabled services.

You will work under the Regional Manager of Innovative Services to help secure nontraditional supports across residential, employment, and community settings, focusing on assessment, planning, linkage, training, and advocacy.

Qualifications

• Must be able to effectively read, write, and communicate verbally in English
• Background check upon hire is required
• Valid driver's license and vehicle insurance
• Experience coordinating services for people with disabilities or related needs

Responsibilities

• Assist individuals to secure nontraditional services, including residential, employment, and community supports
• Provide technical assistance to community providers implementing services
• Advocate on behalf of the person/guardian to reflect the person’s desires
• Conduct follow-ups at 30, 60, and 90 days after transitions to community services
• Follow Tennessee DDA standards and regulations

Innovation Coordinator - Tennessee

Charles Lea Center is looking to hire a full-time Innovation Coordinator in Washington/Surrounding counties.

Please note, you do not have to reside in Washington county but will have to serve Washington county and surrounding counties.

An Innovation Coordinator earns a competitive wage of $22.84-$24.04/Hour. Compensation within the posted pay range is determined based on education and experience.

An Innovation Coordinator (IC) works under the guidance of the Regional Manager of Innovative Services. The primary focus of the IC is to support people to live more effective and meaningful lives by working with them to secure nontraditional services that encompass a combination of staff supports and enabling technologies. This includes residential, employment, and community supports. Key areas include assessment, person centered / MAP's planning, linking people to services, training, and advocacy.

The Charles Lea Center is a CQL accredited organization that is based in Spartanburg South Carolina and provides services in South Carolina, and North Carolina and recently was awarded a contract with the Tennessee Department Disability and Aging (DDA) to provide services in their State. MINIMUM QUALIFICATIONS

• At least 18 years of age
• Must be able to effectively read, write, and communicate verbally in English
• Ability to pass a background check upon hire
• Valid driver's license

• This role involves regular standing, walking, lifting (up to 25 lbs), and hands-on support. Reasonable accommodations are available.

REQUIREMENTS

A bachelor’s degree from an accredited college or university in a human services field;

or

A bachelor’s degree from an accredited college or university in a non-related field and one year of relevant experience;

or

Associate degree and 2 years of relevant experience;

or

High School Diploma or General Equivalent with 4 years of relevant case management experience providing or coordinating services to people with disabilities and special needs and/or with case management services.

*Relevant experience means experience working directly with persons with intellectual, developmental, or other types of disabilities or mental illness.

A DAY AS THE INNOVATION COORDINATOR

• As an Innovation Coordinator, you provide services and supports focused on achieving goals for the people we support.
• Provide technical assistance to Community Providers implementing services for people referred
• Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian.
• Upon successfully transitioning to community services conduct 30, 60, 90 day follow up to assure satisfaction with services and those desired outcomes are being achieved.
• Follow the standards and regulations required by Tennessee Department Disability and Aging (DDA).
