Job summary

Charles Lea Center in Tennessee is hiring a full-time Innovation Coordinator to serve Sumner County and surrounding counties. The IC helps people live meaningful lives by coordinating nontraditional services, including staff supports and enabling technologies.

Under the Regional Manager of Innovative Services, you will assess needs, plan person-centered supports, link people to services, provide training, and advocate.

Qualifications

• A bachelor’s degree in human services or related field, or equivalent experience per the listed options.
• One year of relevant experience if the degree is non-related.
• Associate degree with two years of relevant experience.
• High school diploma with four years of relevant case management experience.

Responsibilities

• Provide services and supports focused on achieving goals for the people we support.
• Provide technical assistance to community providers implementing services for people referred.
• Advocate on behalf of the person/guardian to ensure services reflect the desires of the person/guardian.
• Conduct 30, 60, 90 day follow ups after transition to community services to assure satisfaction and outcomes.
• Follow Tennessee Department Disability and Aging (DDA) standards and regulations.
