The candidate will review orders, monitor drug therapy, and participate in on-call and education activities to maintain high-quality patient care. A PharmD or BS in Pharmacy with active PA licensure is required, plus 2 years of home infusion

Qualifications

• Must have a Doctor of Pharmacy or PharmD with pharmacy licensure.
• Current state-licensed pharmacist; willingness to obtain additional licenses as needed.
• Two years’ recent home infusion therapy experience with specialty meds.
• Ability to evaluate and monitor drug therapy with physicians, nurses and other professionals.
• Strong communication and collaboration with patients and staff; dependable and flexible.

Responsibilities

• Oversee infusion suite patients and coordinate with locations for referrals and admissions.
• Review new orders, verify documentation, and ensure safe and effective therapies.
• Assist physicians and nursing staff to plan and monitor home/infusion drug therapy.
• Monitor therapy outcomes, communicate results to care teams, and maintain records.
• Provide allergy, adverse event and drug therapy counseling as needed.
