• Demonstrates foundation of organizational goals within quality and safety measures.
• Collects, assesses and documents patient healthcare data.
• Involves patient and Interdisciplinary team to identify a realistic plan of care and outcomes.
• Prioritizes care based on data collected.
• Documents assessment and reassessment in medical records.
• Educates patient/family regarding health status and care plan.
• Evaluates progress towards expected outcomes with patient/family involvement.
• Contributes to unit activities to support a healing environment.
