Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care for home and alternate infusion settings under a physician’s plan of care. Licensed and experienced nurse works under supervision to ensure patient safety and continuity of care.

The role involves educating patients and families, documenting care in real time, and coordinating with the clinical team. Travel may be required; strong knowledge of infusion protocols is essential.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit and communicates with physician as needed.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Maintains thorough clinical documentation in real time for billing and continuity of care.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
