Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in home or alternate infusion suites. You will work under supervision to ensure safety and adherence to a physician plan of care.

The role requires an active RN license, CPR certification, and 1+ year of nursing experience. Travel to patient homes is required, and the team prioritizes compliant, compassionate care.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.
• BSN preferred for some roles.

Responsibilities

• Initiates and implements intermediate nursing plans of care and educates patients and families.
• Assesses patient needs at visits and updates physician on changes in condition.
• Provides training and mentorship to Infusion Nurse I as needed.
• Communicates with Clinical team and documents care in real-time.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
• Ensures compliance with policies, procedures, HIPAA, and regulatory guidelines.
• Inserts PICC/Midlines per physician orders and manages line care when applicable.
• Travels to patient homes for infusion care and may cover alternate infusion suites.
