Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an alternate infusion setting. The nurse will work under structured supervision to ensure patient safety and adherence to the physician plan of care.

The role involves assessing patients, administering treatments, educating families, and collaborating with a multidisciplinary team to optimize care and maintain regulatory compliance.

Qualifications

• Must hold a current RN license.
• Experience coordinating home infusion or home health care preferred.
• Ability to educate patients and families for self-management.
• Knowledge of HIPAA regulations and patient confidentiality.

Responsibilities

• Initiates and implements intermediate nursing plan of care in home or infusion suite.
• Assesses patient needs at each visit and clarifies physician orders.
• Provides training to Infusion Nurse I's to ensure patient safety and compliance.
• Educates patients and families to achieve independence with prescribed therapy.
• Communicates with clinical team and physicians to ensure continuity of care.
• Coordinates discharge planning and prepares discharge summaries with clear instructions.
• Complies with policies, procedures, and regulatory guidelines.
