Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an alternate infusion suite, ensuring patient safety under a physician’s plan of care. The role operates under structured supervision and requires a valid RN license and CPR certification.

The position involves initiating nursing plans, assessing patients at each visit, and mentoring Infusion Nurse I staff.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit through health data access and patient interview.
• Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance.
• Provides effective teaching for patient and family to achieve independence with prescribed therapy and care needs.
• Communicates with Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care and documents in real‑time.
• Coordinates discharge planning and prepares discharge summaries with patient instructions and progress reports.
