Job summary

Soleo Health in the Dacula, GA area is seeking a Full-time Infusion Nurse to provide primary patient care for infusion therapy in home or alternate sites, including assessments, care planning, and education.

Responsibilities include direct patient care, coordinating with pharmacists, prescribers and care partners, evaluating home safety, and documenting encounters per policy. On-call rotations and driving to patients are required, with mileage reimbursement.

Qualifications

• Active RN license in good standing.
• BSN preferred.
• Advanced certifications such as CRNI/ONS preferred but not required.
• Willingness to participate in on-call rotation including nights/weekends.
• Experience with infusion therapy and home care.
• Valid driver’s license.

Responsibilities

• Provide direct care for assigned patients, initiating and maintaining communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy.
• Evaluate home environment for safety and appropriateness.
• Develop plan of care with the team and evaluate response to therapy.
• Manage schedule and communicate changes to stakeholders.
• Coordinate patient care via approved company platform.
• Participate in nurse on-call rotation for continuous support.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Ensure timely submission of POTs and clinical documents.
