Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Wheeling, WV region. The role provides primary nursing care for infusion therapy in home or alternate settings with on-call support and travel up to 75 miles as needed.

The ideal candidate will hold an active RN license, have infusion therapy experience, and be willing to obtain a WV multistate license. This position offers competitive wages and mileage reimbursement within Soleo Health's care network.

Qualifications

• Current RN license in good standing.
• Multistate RN license for WV and willing to obtain if needed.
• RN degree, BSN preferred.
• On-call rotation including nights/weekends required.
• Infusion nursing experience strongly preferred (min 3 years).
• ICU or Cardiac Care experience preferred.
• Proficient with Outlook and Office; CPR+ and home infusion tools listed.
• Valid driver's license with no significant restrictions.

Responsibilities

• Provide direct patient care and initiate/maintain communications with care team.
• Educate patient/care partner on home infusion therapy and safety.
• Assess home environment and recommend safety interventions.
• Develop and evaluate the plan of care with the team.
• Manage schedule via company platform and notify of changes.
• Coordinate patient care through approved platforms.
• Participate in the nurse on-call rotation for continuous support.
• Document all patient encounters per policy and regulations.
• Maintain up-to-date knowledge of Soleo therapies and standards.
• Ensure timely submission of POTs and clinical documents; coordinate with departments.
