The Home Infusion Nursing Coordinator is the clinical lead for the Home Infusion Nursing Team, who provides and utilizes clinical expertise and skills to coordinate and provide patient care, patient education and staff training regarding therapy and supplies, as well as clarifying competency and expectations of care in the home. The position will include direct home infusion patient care as needed. This position facilitates support, addresses and triages clinical and administrative queries from the home infusion and pertinent system nursing teams to the appropriate pharmacy and nursing leadership, ensures development and evaluation of competency-based orientation as needed for the Home Infusion Nursing team.

Responsibilities
  • 1. In collaboration with pharmacy and nursing leadership, develops, maintains and grows core therapy and service line admissions to the home infusion pharmacy through positive internal staff and external relationships and provides positive and responsive customer service.
  • 2. Demonstrates thorough knowledge of core therapy referrals and in collaboration with pharmacy and nursing leadership, determines qualifying criteria by payor source safety risk and patient need / requirements to determine referral to admission eligibility.
  • 3. Collaborates with referral sources (i.e. hospital case managers, home health agencies, physician offices) and the home infusion pharmacy staff to facilitate the coordination of care for new patient referrals to the home infusion pharmacy.
  • 4. Coordinates, facilitates and communicates all home infusion-based activities for new patient referrals with the home infusion pharmacy personnel and referral sources as needed. As the clinical lead for the YNHH Home Infusion Therapy team, acts as a nursing team resource and provides direct patient visits as needed. Responsible for the delivery of high-quality care as it relates to delivery of infusion therapy in the home or alternate setting. Facilitates with staff and provides direct to assigned patient and/or caregiver education on proper handling, storage and administration of infusion medications as well as signs and symptoms of access device complications.
  • 5. In collaboration with pharmacy and nursing leadership, plays a significant role in the development, evaluation and implementation of standards of practice and related policies and procedures, ensures compliance with regulatory and accreditation standards relating to nursing.
  • 6. In collaboration with pharmacy and nursing leadership, reviews, escalates and responds to patient and family complaints, investigates unusual events and ensures proper follow-up and documentation.
  • 7. Represents nursing in the development of interprofessional learning activities, acts as education and practice expert and advances organizational initiatives.
  • 8. Supports the development of nursing leadership skills in self and clinical nursing staff according to System, Regulatory and accreditation standards of Practice, to promote professional nursing practice and quality patient care. Mentors and supports staff to develop as professionals through formal / informal education.
  • 9. Attend and participate in appropriate office meetings, sales meetings, training programs and regional and national trade shows and conventions as directed by Nursing and Pharmacy Leadership.
  • 10. Coordination and cross-training of departmental work duties to achieve maximum productivity and efficiency.
  • 11. Stay abreast of developments in field of infusion therapy including new models that support patient care (for example, Ambulatory Infusion Suites).
  • 12. Comply with all company policies and directives for the fulfillment of the company objectives.
  • 13. Communicates any problems directly to the Pharmacy and Nursing leadership.
  • 14. Assesses the learning needs based on regulatory requirements, evidence-based practice, and staff-identified needs.
  • 15. Considers different levels of nursing experience and competency when supporting the nursing team.
  • 16. Acts as education and practice expert in consultation with quality and safety leaders. Represent the home infusion pharmacy in a professional and positive manner.
  • 17. Partners with colleagues across specialties to ensure consistent, high-quality content and education delivery.
  • 18. Uses technology resources to improve quality of education programs.
  • 19. Supports the development of nursing leadership skills in self and clinical nursing staff according to ANA Standards of Practice, to promote professional nursing practice and quality patient care.
  • 20. Mentors and supports staff to develop as professionals through formal / informal education, specialty certification, membership in professional organization, and opportunities for advancement.
  • 21. Acts as a role model to promote the nursing profession.
  • 22. Identifies and responds to learning opportunities based on quality metrics.
  • 23. Gathers, synthesizes, uses, and communicates best evidence to support bedside practice and educational design as appropriate.
  • 24. Utilizes specialty nursing organization standards of practice to ensure education programs remain current and based on best evidence.
  • 25. Develops, implements, evaluates competency-based orientation.
  • 26. Competency Management and Ongoing Education. Develops initial and ongoing competencies in collaboration with nursing education teams.
  • 27. Identifies learning needs to ensure practice competencies in collaboration with nursing unit leadership.
  • 28. Partners to develop continuing education programs.
  • 29. Provides ongoing education programs to support clinical competency growth in response to multiple triggers.
Qualifications

EDUCATION

RN, Bachelor's degree from an accredited college or school of nursing required.

EXPERIENCE

Strongly Preferred: 3-5 years of IV related experience 3 years of home health care experience 2 years of Case Management experience

LICENSURE

Current licensure as a Registered Nurse in the State of Connecticut.

SPECIAL SKILLS

Must have excellent customer service skills (both verbal and written) and have effective interpersonal skills that would allow for positive interactions with varied personality types. Proficient computer skills.
