The Position:

The Infusion Nurse provides primary patient care for specifically assigned patients, including assessments, care planning, evaluation, and education of patients receiving infusion therapy and/or other nursing services in the home or alternate site setting. Responsibilities include:

• Providing direct care for all assigned patients, including initiation and maintenance of communication with care team members such as pharmacists, prescribers, dietitians, and care partners.
• Evaluating and educating patients/care partners to safely administer home infusion therapy and providing additional instruction as needed.
• Assessing the home environment and implementing interventions to ensure safety and appropriateness for prescribed care.
• Developing and evaluating the plan of care in collaboration with team members.
• Maintaining caseload scheduling and communicating scheduling changes.
• Coordinating care via approved platforms.
• Participating in nurse on-call rotation to provide continuous nursing support.
• Documenting all communications per Soleo policy.
• Ensuring all required patient documents are completed and returned timely.
• Submitting treatment plans in accordance with state guidelines.

Schedule:

• 30 hours a week
• 8:30am-3:00pm
• Mandatory on-call 2-3 days per month
• Ability to drive varying distances to patient homes

Requirements:

• RN Degree, BSN preferred
• Optional advanced certifications (e.g., CRNI, ONS, IgRN)
• Participation in on-call rotation, including nights and weekends
• Experience in infusion therapy, ICU, or cardiac care preferred
• Current nursing license for applicable state
• Proficiency with Microsoft Office and Outlook
• Valid driver’s license with no significant restrictions
