UI Community HomeCare Inc. is seeking a Patient Care Coordinator in Iowa City to ensure coordinated infusion and enteral therapy for patients, manage resupply and maintain documentation for billing.

The role requires strong communication, multitasking, and problem-solving abilities within a team including pharmacists, nursing agencies, and insurers. The position emphasizes collaboration with clinical teams and referral sources, with responsibilities spanning prior authorizations, eligibility

Formación

• High school diploma or GED required.
• Experience in healthcare, customer service, or related field.
• Knowledge of EPIC or Brightree preferred.
• Eligibility to work in the United States.

Responsabilidades

• Coordinate care for patients requiring infusion or enteral therapy.
• Communicate with pharmacists to determine drug delivery methods and resolve needs.
• Conduct patient resupply calls and assess supply requirements.
• Obtain prior authorizations and verify insurance coverage.
• Coordinate services with home health or nursing agencies.
• Support accurate billing and participate in quality assurance initiatives.
