Job summary

Ui-Community-Homecare in Iowa City is seeking a Patient Care Coordinator to ensure seamless coordination of infusion and enteral services, manage therapy changes, and oversee the resupply of equipment, medications, and supplies for patients.

You will work closely with patients, caregivers, providers, pharmacies, nursing agencies, and insurers, handling prior authorizations, billing documentation, and compliance with payer guidelines.

Qualifications

Responsibilities

• Coordinate care for newly admitted patients requiring infusion or enteral therapy.
• Communicate with pharmacists to determine drug delivery methods and resolve medication-related needs.
• Conduct patient resupply calls, assessing supply requirements and completing clinical screening questions.
• Ensure compliance with payer guidelines and regulatory standards for resupply processes.
• Obtain prior authorizations and verify insurance coverage to support timely service.
• Identify nursing care needs and coordinate services with home health or nursing agencies.
• Monitor and address irregular supply or medication usage patterns, resolving shipping or service issues as needed.
• Collaborate with referral sources, clinical teams, and inventory departments to ensure efficient and accurate service delivery.
• Participate in quality assurance initiatives, process improvements, and complete required documentation for accurate billing.
• Other duties as assigned.

Position Summary

Education and Experience Requirements

Expected Work Hours and Location
