TwelveStone Health Partners is seeking a Nurse Practitioner to join the infusion center in SW Louisville, KY. The NP will provide comprehensive outpatient infusion care, coordinate treatment plans, and collaborate with a multidisciplinary team to ensure patient safety and quality of care.

Required is a Master's in Nursing, an active APRN license, board certification, and CPR. Infusion experience is preferred, with strong IV skills and patient education abilities.

Qualifications

• Master's degree in nursing from an accredited program.
• Active and unrestricted APRN license in the practice state with board certification (ANCC/AANP).
• CPR certification (BLS minimum) required.
• Infusion experience preferred; oncology/ER/ICU background beneficial.

Responsabilités

• Provide comprehensive outpatient infusion care including assessment, diagnosis, treatment, and care coordination.
• Collaborate with physicians and healthcare teams to ensure high-quality patient care.
• Educate patients and families on treatment plans, safety, and follow-up.
• Document care and maintain accurate records in the EHR system.
• Participate in quality initiatives and program reviews.
