Job summary

TwelveStone Infusion Centers LLC in Louisville, KY is seeking a Nurse Practitioner (Float) to provide comprehensive outpatient infusion care, including assessment, diagnosis, treatment, and care coordination for patients with acute and chronic needs in an outpatient setting. Ideal candidates have an active APRN license and RN license in Kentucky, board certification (ANCC or AANP), and infusion experience.

Qualifications

Master's degree in nursing is required and active APRN/RN license in state of practice with board certification (ANCC or AANP).
CPR/BLS certification required; NCCPA route possible with active license.
2 years NP/PA experience in clinical setting required or 3+ years RN in oncology/ER/ICU/infusion; infusion experience essential.

Responsibilities

Provides patient- and family-centered IV infusion care and coordinates care across the team.
Advocates for patients and families in navigating the health care system.
Communicates and collaborates with physicians and other health professionals; maintains accurate records and participates in quality initiatives.
Under supervision as needed, performs duties to support infusion center operations.
