Job summary

Compassus is seeking a Home Infusion Care Transition Coordinator (Registered Nurse) to model our values and promote seamless post‑acute care transitions. You will collaborate with physicians, hospital case managers, and referral sources to ensure appropriate, timely care for patients moving from acute to home or alternate settings.

Must hold an RN license and have 2+ years of infusion experience; home infusion knowledge and strong communication are essential for success in this role.

Qualifications

• Associate or Bachelor’s degree in Nursing required.
• Active RN license in state of employment.
• Experience in infusion nursing and care transitions preferred.
• Excellent communication and collaboration with physicians, patients, and families.

Responsibilities

• Coordinate home infusion care transitions with physicians, patients, and family caregivers.
• Meet with hospital clinicians to discuss patient needs and ensure continuity of care.
• Educate referral sources and care teams on transition planning and patient goals.
• Develop patient care plans for alternate sites of care and monitor post‑discharge needs.
