Job summary

Compassus in Tampa, FL, is seeking a Home Infusion Care Transition Coordinator. The role requires an RN with infusion experience, focusing on coordinating patient transitions from hospital to home infusion services and aligning care with physicians, discharge planners, and families.

You will analyze post-acute needs, manage care plans, and ensure accurate documentation across teams while promoting Compassus values of compassion and excellence in patient care.

Qualifications

• Associate or Bachelor’s degree in Nursing required.
• Experience in infusion healthcare marketing preferred.
• Minimum of two (2) years' experience in infusion healthcare preferred.
• Sales experience preferred.
• Experience with Medicare Home Infusion Benefit – skilled need, homebound status determination.

Responsibilities

• Regularly meets with physicians in the hospital to discuss specific patients.
• Documents interaction in CPR+; provides guidance and post-acute service support.
• Ensures continuity of care as a priority.
• Communicates data sets to clinicians (e.g., predictive analytics, prognostic scores).
• Develops patient care plans with discharge planners and physicians for transition to home.
• Educates referral sources and caregivers for the transition of patient care from acute setting to home infusion services.
