Job summary

Soleo Health Inc is seeking a Licensed Practical Nurse (LPN) to support infusion patients in Tempe, AZ. The role includes direct patient care, care planning, education, and collaboration with the care team in home settings.

The candidate must hold an active LPN license in Arizona, IV certification, and at least 3 years of nursing experience with infusion therapy. Strong Excel/Word/Outlook skills and excellent customer service are required.

Qualifications

• Current LPN license in good standing
• Must hold IV certification
• Minimum 3 years of nursing experience with infusion therapy
• Proficient with Microsoft Excel, Word and Outlook
• Excellent customer service skills

Responsibilities

• Provide direct patient care for infusion therapy and nursing services in home or alternate site
• Develop and update each patient’s Care Map with the clinical team
• Educate patients and care partners on home infusion administration
• Coordinate care with pharmacists, physicians and home care staff
• Discharge patients and complete discharge summaries within 7 days
• Maintain patient visit schedules and document visits accurately
• Document communications and vital signs, dressings, insertions as required
