Job summary

S Soleo Health Inc. is seeking a Licensed Practical Nurse (LPN) to support infusion patients across Tempe, Avondale, and Glendale in Arizona. The role covers direct patient care, care planning, and education within the home or alternate care sites.

You will work with a multidisciplinary team, manage care maps, and ensure safe, effective infusion delivery. Applicants should have an AZ LPN license and IV certification.

Qualifications

• Current LPN license in Arizona in good standing.
• IV certification as required by state/organization standards.
• Minimum 3 years nursing experience with infusion therapy background preferred.
• Proficient with Microsoft Office tools and EMR documentation.

Responsibilities

• Provide direct care for assigned patients receiving home infusion therapy or nursing services.
• Educate patients and care partners on safely administering infusion therapy at home.
• Assess home environment and ensure safety for prescribed care; adjust care maps as needed.
• Develop and update each patient's Care Map and monitor responses during visits.
• Coordinate scheduling and communications with care team and document visits.
• Discharge patients and complete discharge summaries within 7 days when applicable.

The Position:

The Licensed Practical Nurse (LPN) or Medical Assistant (MA) provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities Include:

• Provide the majority, if not all the direct care for all specifically assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, physicians, dieticians, home patient representatives and any appropriate community resource staff members
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care
• Develop, in conjunction with other clinical team members, each patient's Care Map and evaluate patient's response to therapy during each home visit or encounter
• Appropriately discharge primary patients and complete discharge summary within 7 days of discharge
• Develop and maintain caseload scheduling and communications on a weekly and monthly basis in updating schedule with changes. Schedule home visits with patient/care partner and document on patient visit schedule calendar, which is posted in patient's home
• Document patients visit to physician or lab drawing station on calendar
• Coordinate care via live telephone conversations and/or voicemail with the team as determined by the urgency of information to be relayed
• Document all communications at the time of service
• Assist with vital sign monitoring, PICC dressing changes, PIV insertions, and with administrative needs depending on site of care and state nursing laws

Schedule:

• Full-time, 40 hours per week, Sunday - Thursday, 8:30am - 5pm
• Must be licensed as an LPN in the state of Arizona
• Must be willing to work in Tempe, Avondale and Glendale

Requirements

• Current LPN license for applicable state in good standing
• Must hold current IV certification in accordance with state and organizational standards
• Minimum 3 years of nursing experience with background in infusion therapy preferred
• Proficient with Microsoft Excel, Word and Outlook
• Excellent customer service skills
