Job summary

Soleo Health in Tempe and Phoenix, AZ is seeking a Nurse Liaison to coordinate transitions from hospital to home infusion therapy. You will educate patients and caregivers at bedside, assess clinical needs, and collaborate with discharge planners and physicians to ensure safe discharges.

The Nurse Liaison will work with internal teams to ensure timely logistics, documentation, and smooth patient journeys from hospital to home care. MON-FRI, 8am-5pm.

Qualifications

• Current RN license in good standing is required.
• Minimum 2–5 years of nursing experience; home infusion or case management preferred.
• Experience in patient education and discharge planning is essential.
• Strong clinical judgment, communication, and organizational skills.
• Proficiency with EMR systems and Microsoft Office suite.

Responsibilities

• Coordinate hospital-to-home or alternate-site infusion transitions.
• Collaborate with discharge planners, physicians, and care teams.
• Provide bedside education on IV therapy, device use, and safety.
• Document patient interactions and care plans in the EMR.
• Support marketing by identifying new referral sources.
