Job summary

Old Colony YMCA is seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries. The role focuses on assessing needs and coordinating care for older adults and people with disabilities in the South Shore region.

The position emphasizes developing individualized care plans, evaluating service packages, and monitoring progress. A bachelor’s degree in social work or related field is preferred, with licensure beneficial.

Qualifications

• Bachelor's degree in social work or related field preferred.
• Current Social Work license (LSW) or higher preferred.
• Excellent written and verbal communication skills.
• Strong computer skills and proficiency with Microsoft Office.
• Ability to work within an interdisciplinary team.

Responsibilities

• Assess the health and functional status of members.
• Develop community-based care plans and service packages.
• Participate in development of a member's ICP.
• Arrange, coordinate, and monitor provision of services.
• Assist in determining need for institutional and community long-term care.
