Job summary

Qualifications

• Bachelor's degree in social work or related field; licensure preferred
• Minimum of two years' professional experience with adults 65+
• Knowledge of aging process and ability to interview clients
• Ability to gain trust of clients and families
• Ability to work within an interdisciplinary team

Responsibilities

• Assess the health and functional status of members
• Develop community-based care plans and service packages
• Participate in development of a member's ICP
• Arrange, coordinate, and monitor provision of services
• Determine need for institutional and community long-term care

We are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.

What you'll do

• Assess the health and functional status of members
• Develop community- based care plans and related service packages
• Participate in the development of a member's Individual Care Plan (ICP)
• Arrange, coordinate, and monitor the provision of appropriate members
• participate in the determination of a member's need for institutional and community long term care services

What you'll bring

• Your passion for helping others and desire to make a difference.
• Bachelor's degree in social work or related field highly preferred; current licensure if appropriate
• Minimum of two years' professional experience in the care of people 65 and older
• Current Social Work license (LSW) or higher preferred
• Working knowledge of social, emotional, and physical process of aging
• Ability to obtain information of a personal nature through interviews
• Ability to gain and hold the confidence of clients and their families
• Draw appropriate conclusions and make recommendations
• Knowledge and skill necessary to work successfully within an interdisciplinary team
• Excellent written and verbal skills.
• Excellent computer skills.
• Proficient with Microsoft Office Suite or related software.
