Job summary

Old Colony Elder Services (OCES) is expanding its South Shore service area and seeks an Information & Referral Specialist to assist consumers with service and eligibility inquiries. The role focuses on assessing needs, coordinating community-based care, and developing individualized care plans to promote independence and safe living.

The position requires a related degree, a social work license or higher, and at least two years’ experience in care of older adults.

Qualifications

• Bachelor's degree in social work or related field is highly preferred; current licensure if appropriate.
• Minimum of two years' professional experience in the care of people 65 and older.
• Current Social Work license (LSW) or higher preferred.
• Excellent written and verbal skills; proficient with Microsoft Office Suite.

Responsibilities

• Assess the health and functional status of members.
• Develop community-based care plans and related service packages.
• Participate in the development of a member's ICP.
• Arrange, coordinate, and monitor the provision of appropriate members' services.
• Participate in the determination of a member's need for institutional and community long-term care services.

OCES is growing! We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc.

We are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.

What You'll Do

• Assess the health and functional status of members
• Develop community- based care plans and related service packages
• Participate in the development of a member's Individual Care Plan (ICP)
• Arrange, coordinate, and monitor the provision of appropriate members
• participate in the determination of a member's need for institutional and community long term care services

What You'll Bring

• Bachelor's degree in social work or related field highly preferred; current licensure if appropriate
• Minimum of two years' professional experience in the care of people 65 and older

• Working knowledge of social, emotional, and physical process of aging
• Ability to obtain information of a personal nature through interviews
• Ability to gain and hold the confidence of clients and their families
• Draw appropriate conclusions and make recommendations
• Knowledge and skill necessary to work successfully within an interdisciplinary team
• Excellent written and verbal skills.
• Excellent computer skills.
• Proficient with Microsoft Office Suite or related software.
