Job summary

UTMB Health in Galveston, TX seeks a Pharmacy Clinical Practice Specialist in Infectious Diseases to provide advanced medication therapy management and direct patient care across practice areas. The role collaborates with physicians, nurses, and other professionals to optimize therapy, ensure safety, and align with system goals.

Requires PharmD or B.S. in Pharmacy and Texas licensure or eligible for licensure, with PGY1 residency or 3 years related experience.

Qualifications

• Licensed to practice pharmacy in Texas or eligible for licensure by Texas State Board of Pharmacy
• Must be licensed within 90 days of hire by Texas State Board of Pharmacy
• Completion of ASHP-accredited PGY-1 residency or 3 years of related experience
• Preferred: Doctor of Pharmacy degree
• Preferred: ASHP-accredited PGY-2 residency in Infectious Diseases
• Preferred: BCIDP certification within 12 months of start date

Responsibilities

• Assess medication regimens and optimize outcomes and cost-effectiveness
• Monitor and evaluate therapy and adverse events; adjust per protocol or with prescribers
• Implement evidence-based guidelines and regulatory requirements
• Collaborate with physicians, nurses, case managers and other clinicians
• Participate in patient care rounds, case conferences and interdepartmental meetings
• Serve as a clinical resource for drug information and policy interpretation
• Support medication-use safety and distribution by coordinating with operations
• Identify and report medication-use system issues needing quality/safety actions
• Educate patients on medications, adherence, and safety
• Precept learners and participate in experiential education
• Develop and refine clinical pathways and protocol-driven services
• Support compliance with accreditation and regulatory standards
