Chesapeake Regional Healthcare in Chesapeake, VA is seeking a Clinical Pharmacy Specialist I - Infectious Diseases to lead pharmacotherapy services within the ID specialty. The role requires advanced clinical judgment, autonomy, and collaboration with physicians, nurses, and pharmacists to optimize antimicrobial therapy, monitor safety, and implement evidence-based protocols.
A PGY2 in infectious diseases is preferred.
Competenze
- ACPE-accredited pharmacy degree required.
- Doctor of Pharmacy or equivalent advanced degree preferred.
- ASHP PGY1/PGY2 residency in infectious diseases is preferred.
Mansioni
- Establish and maintain infectious disease pharmacotherapy services.
- Evaluate regimens for indications, dosing, and interactions.
- Monitor safety and efficacy of therapies and intervene as needed.
- Lead hospital committees and provide education to staff.
- Develop policies and guidelines for medication use.
- Precept residents and students; participate in quality improvement.
