Baystate Health is seeking an Infectious Diseases Clinical Pharmacy Specialist to lead and provide comprehensive pharmaceutical care. The role emphasizes antimicrobial stewardship, education, and collaboration with the care teams within Baystate Medical Center.

The position requires a PharmD, PGY-1/2, and Board Certification in Pharmacotherapy, with an expectation to pursue BPS certification within 12 months. Onsite in Springfield, MA, with a strong focus on patient outcomes and education.

Qualifications

• PGY-1 or equivalent to two years in a direct patient care role.
• Board Certified Pharmacotherapy Specialist - Board of Pharmacy Specialist
• Doctorate in Pharmacy

Responsabilités

• Provide comprehensive and collaborative pharmaceutical care for infectious diseases patients.
• Lead antimicrobial stewardship and related programs.
• Precept APPE students and pharmacy residents; mentor staff.
• Develop and implement evidence-based drug therapies and clinical guidelines.
• Obtain any applicable BPS certification within 12 months of hire.
