Job summary

Stormont Vail Health in Topeka, KS is seeking a Registered Nurse Case Manager for Infectious Disease to assess patients and coordinate services across care transitions.

BSN preferred; minimum 5 years acute care nursing and strong assessment, communication, and advocacy skills are required.

Qualifications

• 5 years acute care nursing experience in a hospital setting.
• Quality management/Joint Commission, discharge planning and/or case management experience preferred.
• Experience conducting clinical assessments and coordinating home care services.

Responsibilities

• Analyzes assessment data to determine problems and initiates prior authorizations for medications and treatment plans.
• Facilitates coordination of care for ambulatory services including infusion, wound care, and dialysis.
• Leads discharge planning with the care team for patients transitioning to new levels of care.
• Educates patients and families about care options and resources to support treatment decisions.
