Job summary

Stormont Vail Health in Topeka is seeking a Hospitalist Case Manager to support adult patients during hospitalization. This role coordinates discharge planning, ensures timely care transitions, and documents care summaries for physician review at discharge.

The ideal candidate has a BSN (preferred), 5 years of acute care nursing experience, and strong critical thinking, communication, and organizational skills.

Qualifications

• BSN preferred.
• 5 years acute care nursing preferred.
• Experience with discharge planning and case management preferred.

Responsibilities

• Facilitates coordination of care and discharge planning for patients transitioning to the next level of care.
• Communicates with health care team about discharge needs and plans.
• Maintains relationships with community partners across the continuum of care.
• Keeps supervisor informed of quality and risk issues.
• Identifies areas for process improvement and participates in related activities.
• Identifies barriers to achieving goals in the plan of care and resolves them using critical thinking.
• Represents the Hospitalist team to patients and families.
• Educates and facilitates primary nursing regarding acute patient medical management.
• Ensures completion of discharge paperwork and referrals.
• Coordinates post-discharge follow-up care with accepting facility or primary care.
