Job summary

Stormont Vail HealthCare, Inc. is seeking an RN Case Manager for Infectious Disease to assess patients for needed services and coordinate care across the continuum, including discharge planning and post-discharge support.

You will collaborate with the health care team on medically complex cases, provide resources to patients and families, educate them about treatment options, and ensure regulatory compliance in a fast-paced hospital setting.

Qualifications

• Bachelor's degree in nursing (BSN preferred).
• Minimum 5 years of acute care hospital nursing experience.
• Experience with quality management, discharge planning and case management.
• Clinical assessment skills and ability to set up home care services.

Responsibilities

• Analyzes assessment data to identify problems and needs.
• Coordinates care and discharge planning for transitioning patients.
• Communicates with the health care team about concerns or recommendations.
• Provides resources to educate patients and families about care options.
