Stormont Vail Health is seeking a Hospitalist Case Manager to coordinate inpatient care and discharge planning for adult patients, working with the hospitalist team to ensure seamless transitions.

The role requires a BSN, Kansas RN licensure (KSBN) or multistate license, and at least 5 years of acute care experience, with emphasis on discharge planning and care coordination across settings.

Qualifications

• RN licensed in Kansas (KSBN) or multi-state license accepted
• Minimum 5 years acute care nursing experience in a hospital setting
• Preferred discharge planning and case management experience
• Experience conducting clinical assessments and setting up home care services

Responsabilités

• Coordinate care and discharge planning with the health care team for patients transitioning to the next level of care
• Communicate with the health care team regarding discharge needs or plans
• Maintain relationships with community partners across the continuum of care
• Inform supervisor of quality and risk issues and participate in process improvement
• Delegate tasks to health care team members while ensuring patient care quality
