Qualifications

• BSN preferred for RN licensure and practice.
• 2 years nursing experience in an acute or ambulatory setting required.
• Case/care management experience preferred.

Responsibilities

• Participates in clinic redesign and PCMH expansion to optimize care coordination.
• Establishes collaborative relationships with primary care practices and care team members.
• Initiates communication with high-risk patients identified for targeted care.
• Pulls data from multiple systems to create a performance picture and guides interventions.
• Manages care transitions and discharge planning in collaboration with care teams.
