Job summary

Trio Health Partners is seeking an In-House Infectious Disease Nurse Practitioner to lead ID care within skilled nursing facilities. You will serve as the primary provider for residents' infectious disease plans, collaborating with facility staff and families to drive proactive outcomes.

Qualifications

• Active Illinois Nurse Practitioner license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience.

Responsibilities

• Serve as the primary ID NP for residents in facilities.
• Collaborate with facility staff and providers on care plans.
• Lead clinical activities as part of the leadership team.

As anIn-House Nurse Practitioner specializing in Infectious Disease, you will be the primary provider helping your facilities clinical expertise in their Infectious Disease programs by participating in appropriate resident's plan of care. You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

At Trio Health Partners, we focus on preventing critical health issues before they arise, delivering proactive care to residents in our Skilled Nursing and Long-Term Care facilities.
