Job summary

Trio Health Partners in Illinois seeks an In-House Nurse Practitioner specialized in Infectious Disease to lead and support ID programs across skilled nursing facilities, partnering with physicians and care teams.

You will perform exams, prescribe treatments, develop policies, coordinate discharge visits, and ensure timely documentation, with a focus on preventing hospitalizations and delivering proactive geriatric care.

Qualifications

• Active Illinois Nurse Practitioner License (unencumbered)
• Master's Degree in Nursing from an accredited program
• Strong clinical background in Infectious disease
• Geriatric or adult medicine experience in skilled nursing/long-term care settings
• Ability to conduct 12 patient visits per day with bonuses for exceeding target

Responsibilities

• Perform comprehensive physical examinations and evaluate changes in patient conditions
• Prescribe medications and treatments as needed
• Develop patient care policies and facilitate referrals to specialists
• Coordinate advanced care planning and discharge visits
• Ensure timely communication and documentation of assessments, treatment plans, and follow-ups
• Support physician-patient relationships and address gaps in care

As an In-House Nurse Practitioner specializing in Infectious Disease, you will be the primary provider helping your facilities clinical expertise in their Infectious Disease programs by participating in appropriate resident's plan of care. You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities:

As the In-House Nurse Practitioner specializing in Infectious Disease, you will provide essential care and leadership within the facility, including:

Salary & Compensation:

• Annual Pay: $105,000 - $130,000 based on experience
• Monthly Bonus Opportunities: Monthly performance-based bonuses
