TRIO HEALTH PARTNERS in Illinois seeks an Infectious Disease-focused Nurse Practitioner to lead clinical care across skilled nursing facilities. You will serve as a primary provider, building strong relationships with residents, families, staff, and physicians.

Qualifications

• Active Illinois NP license (unencumbered).
• Master's in Nursing from an accredited program.
• Strong clinical background in infectious disease.
• Geriatric or adult medicine in skilled nursing/long-term care.
• Ability to manage 12 patient visits per day with bonus potential.
• Knowledge of Medicare/Medicaid and health plan benefits is a plus.

Responsibilities

• Perform comprehensive physical exams and evaluate patient changes.
• Prescribe medications and treatments as needed.
• Develop care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely communication and documentation of assessments and plans.
• Support physician-patient relationships and address gaps in care.

Infectious Disease Nurse Practitioner (NP)

As an In-House Nurse Practitioner specializing in Infectious Disease, you will be the primary provider helping your facilities clinical expertise in their Infectious Disease programs by participating in appropriate resident's plan of care. You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:

• Active Illinois Nurse Practitioner License (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease.
• Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.
• Ability to engage and communicate effectively with elderly and chronically ill patients and their families.
• Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.
• Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities:

As the In-House Nurse Practitioner specializing in Infectious Disease, you will provide essential care and leadership within the facility, including:

• Performing comprehensive physical examinations and evaluating changes in patient conditions
• Prescribing medications and treatments as needed
• Developing patient care policies and facilitating referrals to specialists
• Coordinating advanced care planning and discharge visits
• Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups
• Supporting the physician-patient relationship and addressing gaps in care

Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.

Salary & Compensation:

• Annual Pay: $105,000 - $130,000 based on experience
• Monthly Bonus Opportunities: Monthly performance-based bonuses
