Job summary

Trio Health Partners is seeking an In-House Nurse Practitioner specializing in Infectious Disease to lead clinical efforts in our geriatric facilities. You will be the primary provider for ID programs, shaping plans of care and collaborating with residents, families, and staff.

Qualifications

• Active Illinois NP license (unencumbered).
• Master's degree in nursing from an accredited program.
• Strong clinical background in infectious disease.
• Geriatric or adult medicine experience.

Responsibilities

• Serve as the primary Infectious Disease NP in skilled nursing facilities.
• Participate in residents' plans of care and clinical decision making.
• Build lasting relationships with residents, families, facility staff and providers.
• Contribute to the clinical leadership team and preventive care initiatives.

As anIn-House Nurse Practitioner specializing in Infectious Disease, you will be the primary provider helping your facilities clinical expertise in their Infectious Disease programs by participating in appropriate resident's plan of care. You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

At Trio Health Partners, we focus on preventing critical health issues before they arise, delivering proactive care to residents in our Skilled Nursing and Long-Term Care facilities.
