Job summary

AndHealth is seeking a Care Navigator Nurse (RN or LPN) for our Columbus, Ohio team. This full-time role focuses on infectious disease patients, guiding them to high-quality care and outcomes.

You will travel to CHCs and other sites, room patients, record vitals, assist procedures, document in the EHR, coordinate referrals and tests, and support front-office tasks while upholding AndHealth values. A strong communicator with experience in infectious disease care is ideal.

Qualifications

• Active Ohio LPN or RN license in good standing.
• Associate degree in nursing required; BSN preferred.
• BLS certification required or willingness to obtain after joining AndHealth.

Responsibilities

• Travel to CHC locations to meet patients where they are.
• Prepare exam rooms and ensure supplies and cleanliness.
• Room patients, check vitals, and communicate needs to the provider.
• Assist with patient procedures as directed.
• Document in the EHR in a timely manner.
• Coordinate referrals and test scheduling.
