Job summary
Trio Health Partners is seeking an Infectious Disease Nurse Practitioner to serve as the primary provider within our geriatric-focused facilities. The role focuses on coordinating Infectious Disease programs, collaborating with nursing staff and physicians, and delivering proactive care in skilled nursing and long-term care settings.
You will build strong relationships with residents and families, meet a target of patient visits, and participate in leadership activities within the clinical team
Qualifications
- Current Illinois NP license (unencumbered).
- Master's degree in Nursing from an accredited program.
- Strong clinical background in Infectious disease.
- Geriatric or adult medicine experience in skilled nursing settings.
- Ability to see at least 12 patients per day with bonuses for higher.
- Knowledge of Medicare/Medicaid benefits is a plus.
Responsibilities
- Perform comprehensive physical exams and assess changes in patient conditions.
- Prescribe medications and treatments as needed.
- Develop care policies and coordinate referrals to specialists.
- Coordinate advance care planning and discharge visits.
- Ensure timely documentation and follow-ups.
- Collaborate with physicians and lead in patient care within facilities.
