Job summary

Aviata Health Group is seeking an In-House Nurse Practitioner specializing in Infectious Disease to lead clinical care in our skilled nursing facilities. You will work with residents, families, facility staff, and providers to deliver proactive, high-quality care while reducing rehospitalizations.

We offer work-life balance with flexible full-time schedules, no nights or weekends, reimbursed licensure, CME support, and a two-way EMR system to streamline charting.

Qualifications

• Active Florida NP license (unencumbered).
• Master's Degree in Nursing.
• Strong infectious disease clinical background.
• Geriatric or adult medicine experience in SNF/long-term care.
• Able to see 12 patients per day; bonuses for exceeding.
• Knowledge of Medicare/Medicaid/Health Plan benefits is a plus.

Responsibilities

• Perform comprehensive physical examinations and monitor changes in patient conditions.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely communication and documentation of assessments and plans.
• Support the physician-patient relationship and address gaps in care.
