Job summary

Trio Health Partners is seeking an Infectious Disease Nurse Practitioner (NP) to lead infectious disease care within skilled nursing facilities in Illinois. This in-person, part-time role emphasizes proactive patient care, collaboration with the clinical team, and strong resident relationships.

Ideal candidates will hold an Illinois NP license, a Master's in Nursing, and have infectious disease expertise with geriatric focus.

Qualifications

• Active Illinois Nurse Practitioner license (unencumbered).
• Master's Degree in Nursing from an accredited program.
• Strong clinical background in Infectious disease; geriatric or adult medicine experience preferred.

Responsibilities

• Perform comprehensive physical examinations and evaluate changes in patient conditions.
• Prescribe medications and treatments as needed.
• Develop patient care policies and facilitate referrals to specialists.
• Coordinate advanced care planning and discharge visits.
• Ensure timely communication and documentation of patient assessments and follow-ups.
• Support the physician-patient relationship and address gaps in care.
